DELIVERING CHANGE TOGETHER: Celebrating integrated care innovation
Two years of hard work and innovation came to life at the recent Delivering Change Together - CHN Programme shared learning webinar, a milestone event showcasing progress, collaboration, and integrated ways of working across Louth and Meath.
The webinar spotlighted four standout service improvement projects led by multidisciplinary teams from Community Healthcare Networks (CHNs), supported by developmental partners Organisation Development and Design, Leadership Learning and Talent Management (LLTM), PMO, and Digital Health:
Developing a MDT triage process to streamline multidisciplinary referrals.
Enhancing GP–CHN communication through a digital platform (Padlet).
Creating a guidance document for group work to support service delivery.
Launching a video initiation project to provide digital information tools for service users.
With 170 participants representing disciplines from administration, dietetics, nursing, occupational therapy, physiotherapy, podiatry, social work, speech and language therapy, and GP leads, the programme demonstrated the power of collaboration in driving integrated healthcare.
Reflecting on the success of the partnership, Sinéad Molloy, Organisation Development Officer, said, “Supporting change as one team takes intentional collaboration. It’s not just about working alongside each other, it’s about aligning around a shared purpose and building trust. When we create a space of shared purpose and psychological safety, we don’t just help teams deliver integrated care - we show what integrated working really looks like.”
Cecily Roche, Regional HSCP Integration Development Lead, introduced her new role, which will provide strategic clinical leadership across 26 disciplines, advancing integrated working and career progression through initiatives such as Advanced Practice.
In the panel discussion, Carol McCann, Digital Health, reminded attendees that while technology is transforming health services, “Technology is the last part of the puzzle. Solving problems with the right people in the room is the priority.”
Commissioned by CHN managers in Louth and Meath, the programme recognises that delivering integrated care requires a big shift in mindset and practice - embracing multidisciplinary teams and new ways of working.
